Please find a link to the Healo app for individual doctors’ New Patient Forms at the bottom of this page. Please complete all required information before your first visit, as this will allow us to focus more on your health than on administration.
Preparing for Your First Appointment:
For your initial visit, please arrive 30–45 minutes early and bring your driver’s license/photo ID and insurance card. This additional time allows our front office to complete your registration process smoothly, and we appreciate your patience.
Appointment Confirmation and Cancellations:
Out of respect for all our patients, please confirm or cancel your appointment at least one business day in advance. Cancellations made less than 24 hours prior will incur a cancellation charge of up to $125.00. You can cancel or reschedule by calling us at 214-692-8541 or messaging us through the patient portal. If necessary, you may leave a voicemail.
Wellness Program:
If you are meeting with a prevention coach, please arrive 5 minutes early and wear appropriate clothing for moderate exercise. The program has an initial fee of $125 for first-time visits, with applicable copays thereafter. There is a $60 cancellation fee for onsite training and a $45 cancellation fee for online nutritional visits. Payments can be made through the Healo app or at your appointment.
Our Office & Policies:
– Parking: Free parking is available, and our office is handicap accessible.
– Hours: We’re open from 7:30 AM to 5:00 PM, Monday through Friday. Calls are answered 24/7. After-hours calls will be directed to the doctor on call for emergencies.
– Emergencies: In an emergency, please call 911 or go to your nearest hospital.
We are happy to accommodate your urgent needs whenever possible. However, please note that if you arrive late, it may not be possible to be seen. Payment for office services is due at the time of your appointment and can be made via Visa, MasterCard, Discover, American Express, cash, or check.
