New Patient Information

Dear Patient:

Medical Specialists Associated would like to welcome you as a patient to our office. At the bottom of this page you will find registration forms. Please complete all necessary information on these forms prior to coming in for your appointment. You may bring the forms with you and/or fax them to 214-242-1035 prior to your appointment. Please arrive 15-30 minutes early for your first appointment and bring your drivers license/photo ID and insurance card with you. We appreciate your early arrival to allow time for preparing your file. You must confirm your appointment 24 hours in advance.

We have convenient free parking in the front of our office and we are handicap accessible. The office hours are from 8:30 am to 5:00 pm, Monday through Friday, and telephones are answered 24 hours a day, 7 days a week. After hours and weekend calls will be given to the doctor on call, who is available for emergencies. If you require immediate care during these hours, please go to the Emergency Department at Presbyterian Hospital, or a network hospital specified by your insurance.

Appointments must be made in advance and cancellations at least 24 hours in advance. However, if your condition is emergent, we will make every effort to schedule you as soon as possible. If you are late for an appointment, it may not always be possible to see you. Office services are payable at the time of service. Payment can be made by Visa, Master Card, Discover, American Express, cash or check.

Our office is staffed with well-trained professionals. Our desire is to serve you in a courteous and efficient manner. Phone calls are carefully and thoroughly evaluated to ensure accurate and efficient handling of your important questions and concerns. Every effort will be made to return your call promptly. Your cooperation in this process allows us to serve you to our maximum ability.

  • Office Services are payable at the time of the service
  • You will be provided with an itemized receipt at the end of your visit which contains all of the information your insurer will need to process your claim. Just fill out the personal information requested on the claim form which your insurance company provides to you and mail the form along with the insurance copy of the itemized receipt
